British submarine: HMS Supreme

I would like to know which one was the last action of the British submarine "HMS Supreme" and where.

Apparently it was in service in the Bengala Bay in the beginning of 1945. Did this sumarine take part later in any operation near Japan at the end of the war?? Does someone know when did the submarine return to the UK?

From "U.S. Submarine Attacks during World War II" by John D. Alden, which includes UK submarine attacks in South East Asia:

HMS Supreme conducted 5 war patrols under command of T. E. Barlow (Rank not listed, but probably a Lt.). Patrols 1 (Nov 44) and 2 (Jan 45) in vicinity of Andaman Islands in Bay of Bengal. Patrol 3 (Feb 45) east of Sumatra (Straits of Malacca?). Patrols 4 (May 45) and 5 (July 45) in Gulf of Siam. No torpedo attacks, surface gun actions only against small craft such as wooden coasters and sailing junks. Only named ship attributed was auxiliary nettender Agata Maru, 302 GRT, sunk south of Ross Island (Andaman Islands?) on 19 Jan 45.

I don't believe any British submarines took part in any operations near Japan.
