help with translation

In my father's Soldbuch there is a list of recipients that I would like the proper translations for:

-1- Soldbuch.
-1- Wehrstammbuch.
-1- Gesundheitsbuch,
-1- Verwendungskarte.
-5 Kriegsstammrollenblätter

Any help will be appreciated.

Are there any better source of military translations than TM-30-306 and Special Series No. 12 German Military Translation?
