I don't think a decision can be made with a photo. Very few real and mostly fakes. Some documentation in Waffen Journal, an older periodical published in Germany. I would look for documentation on this rather than opinions.

A quick google search of "Nazi Belt Buckle Gun" produced these results.

Weapons exempted from the US NFA as curios and relics:German, (Nazi) Belt Buckle Gun, .22 rimfire, marked "DRP Ausl Pat, Louis Marquis, W. Elberfeld." German, (Nazi) Belt Buckle Gun, 7.65mm, marked "D.R.P. Angem."

Be inventive and search the web if you don't wish to look for relevant books in your library. This is a great web site, but there are others dealing more with such firearms.

SS Belt Buckle Guns

Several years ago I spent some time on these interesting belt buckle pistols. I had the SS four-barrel specimen in .22 (photo attached) in my possession and tried to gather as much information as possible. I’ve seen photos of these devices in varying calibers, number of barrels, and branch of service. Some were profusely engraved; all had single digit serial numbers, and bore Army, Luftwaffe, SS or Political cap eagles on the front.
I uncovered two lengthy magazine articles that discuss these belt buckle pistols:
GUNS & AMMO, April 1978, p.31, BELLY GUNS OF THE SS
GUNS & AMMO, June 1995, p.98, BELT BUCKLE PISTOL
Obviously, there has been much debate about whether these were official Third Reich production items. However, most consider these interesting and exotic belt buckle pistols as having been hand crafted in postwar machine shops in Germany in the 50’s and 60’s. But, the jury is still out.
