I found this in my grandmother's possessions. She lived in Sydney, Australia. Anyone know what it is?

It's a medal from Panama–Pacific International Exposition, San Francisco 1915.

Not just a medal / coin but both sides and two bears. When the coin is “flipped or spun” and turns around front to back fast, the bears become one (optically) mating pair.
Try a search for “1915 PPIE Naughty Bears”.
