Fürsorge Ausweis

What purpose had a Fürsorge Ausweiss? and why would it have an number? (or just german administration).

Most likely as proof that the holder was working for the "Für[...]sorge" = für jemanden sorgen = to care for someone. Mostly located at the district authorities and nowadays in Austria called "Referat Jugendwohlfahrt und Soziales" = department for youth welfare services and social welfare".
ID cards of officials have always a number written on them.

Could it also be that he was medicaly checked?

I asked also the question in another forum and apparently the Fürsorge ausweiss was given to the foreign volunteers so the family could get an favourable treatment by the government.

it is no card just yes and a number.

Info from mid 1940:

"Roter Ausweis" zur bevorzugten Abfertigung vor Amtsstellen.
Die Versorgung und Fürsorge für Kriegsbeschädigte und Hinterbliebene

Red ID card for preferential processing before official offices.
The care and welfare for war victims and survivors.

Halbe Eintrittspreise für Schwerkriegsbeschädigte
