The form was for a Johannes Fritz Boysen and has a date of February 24th 1936. He was born on December 3rd 1907 in Flensburg. He held the rank of Feldwebel and served with 8th Machine Gun Company of the 27th infantry Regiment in Rostock.

The document list the Name, birth dates, place of birth and religion of his mother, father, Grandmother on his mothers and fathers side and grandfather on his mothers and fathers side.

On the bottom of the form it reads:
