I am unable to decipher the cursive handwriting in the attached document. Also any idea about the stamp. Thanks for any help

Alan, that's my take on it, not sure if the words are correct.
As the scan was in greyscale only, the annotation in color did not show up right.
And I forgot to put a dash after the 7 and 8 on top.
