Translating a German Document

I requested a document regarding a possible birth to my grandfather & his first wife. This was sent, but I am unable to read, especially the handwriting in the margin. Any help is appreciated. Dorothy

Vor dem unterzeichneten Standesbeamten erschien heute, der Persön-
lichkeit nach durch sein Familien-Stammbuch
anerkannt,
der Bankbeamte
Wilhelm Meyer,
wohnhaft in Zoppot, Danzigerstraße 18,
ewangelischer Religion, und zeigte an, daß von der
Christliebe Meyer geborenen Bromberg,
seiner Ehefrau,
ewangelischer Religion,
wohnhaft bei ihm,
zu Zoppot in seiner Wohnung,
am zwölften Mai des Jahres
tausend neunhundert sechs und zwanzig, Nachmittags
um zehn Uhr ein Mädchen
geboren worden sei und daß das Kind einen Vornamen
noch nicht
erhalten habe.

Vorgelesen, genehmigt und unterschrieben.

Because name of the newborn was not provided on the 15th, on the 17th a shortened version of the main document was handwritten on the right side and this time given names were: Edith Christel Charlotte
On behalf of the Registrar document on the right side was signed by von Ostrowski.

Here is what I read on the right side.
Hiltraut

Zappot, Danzigerstraße 18,

nebenbezeichneten Kinde
die Vornamen Edith Christel
Charlotte beigelegt worden
seien.

unterschrieben.

Thank you also Greg. Your help is so appreciated. I am impressed that you were able to read the handwritten portions. The script was similar to how my mom wrote.
