I'm not sure if this is also considered "translation help" as it's defined in this channel, so sorry if this is the wrong place to post this.
After about a year and some months, I was able to establish contact with the Bundesarchiv and they sent me some request forms to fill out to send to Selke GmbH for photocopies of some historical documents.
I'm not sure what some of the fields mean on these request forms, and also which ones I, as the customer, should fill out, and which ones I should leave blank? Also, since the names of the fields are a bit confusing, I'm not quite sure which information goes where, or even what they're asking for?
I've attached the pictures of the sections of the form in question below, and the files they're related to (the files that i contacted the archives about in the first place). I'd appreciate any help I could get regarding this.
