In March 1918 in a highest of all-order of S.M. emperor Wilhelm as medical officer of the army inspecting the ambulances in Sofia and Konstantinopel. This was the camouflage for a secret mission to king Ferdinand of Bulgaria and to the Turkish Sultan (intercepted by marshal Otto Liman von Sanders-Pascha, who smoothing the access to court. Sauerbruch received a high Turkish award and the Hausorden von Hohenzollern.
1918 organized the german medical service in the internment camps in Switzerland (also exchanging procedure).

21.02.1919 Sauerbruch saved the murder of the left-socialist and agitator Kurt Eisner. Soon after that a central council of the revolutionaries had sentenced to death the assasin Anton Graf von Arco auf Valley and demanded the extradition.
Refusing this Sauerbruch was captured by the “Reds” and had a narrow escape from his own death-warrant.
After the fall of the “Münchner Räterepublik“ (Soviet Government) Sauerbruch concealed at Tilla Durieux’ (famous actress) request material severely incrimating the Jewish dramatist Ernst Toller (* 01.12.1893 in Samotschin / Poland; Abschnittskommandant der "Roten Garde" in Munich; five years confinement in a fortress because of high treason; 22.05.1939 Suicid in New York).
Sauerbruch began to develop his “Umkipp-Plastik“ (titing over method), i.e. the compensation of the striked femur-bone by the bone of the amputated lower leg.

22.08.1942 inspecting visit to the Southern area of the Eastern front to Stalino and Krasnodar; but before he should fly as planed to the Crimea he was recalled by Hitler. He was sent per Ju 52 for the Turkish Foreign Secretary Menemencioglu (2 operations)
24.05.1943 - 26.05.1943 Sauerbruch sat through a lecture (“3. Arbeitstagung der Beratenden Ärzte Ost“) at the Berlin Military Medical Academy given by two of the most notorious of the doctor-killers, SS-Gruppenführer und Generalleutnant der Waffen-SS Prof. Dr. med. Karl Gebhardt and SS-Sturmbannführer Dr. med. Fritz Fischer, on the subject of gas gangrene experiments on prisoners. Sauerbruch’s only argument on this occasion was that surgery was better than sulfanilamide! (In “Das Diktat der Menschenverachtung. Eine Dokumentation“ Mitscherlich und Mielke reported in 1947 that Sauerbruch was sanctioning human experiments with condemned to death “capital-criminals being at disposal voluntarily. Then Sauerbruch and others effected per temporary decree from 29.07.1947 by the 13th Zivilkammer of the Landgericht Berlin this text had to be absent in all reprints. In lieu of an oath he declared on 14.06.1947 nearly all performances were concerning to the sulfanilamides he wanted to conteract their overrating.

22.07.1944 after the plot Sauerbruch was interrogated twice by Kaltenbrunner himself, also because of his many connexions to the resistance-circle. Once he drove Generaloberst Beck to a confidential conversation with General Olbricht.
His son, the Knights Cross-holder Peter Sauerbruch, also a “Bamberger Reiter” as Stauffenberg, was brought to the cellar of the Prinz-Albrecht-Straße in September 1944, but his father already could go to meet him in Kaltenbrunner’ s office. SS-General Prof. Dr. med. Karl Gebhardt, being his student in Munich, seems to have helped the old Geheimrat. Peter Sauerbruch was obviously rescued by Alfred Jodl and Heinz Guderian (who would call him back as Ia of the 4th Panzer-Division).
In Belgium Sauerbruch saw evidently trenches built for the new V 1-weapons. The oppostional consulate-secretary Fritz Kolbe, the husand of Sauerbruch’ s secretary, informed the OSS in Bern about that.
01.05.1945 the Soviets conquered the operating shelter of the Charité in Berlin.
In May 1945 manager of the Public Health Adinistration in whole Berlin, his deputy was the former Generalarzt der Reserve Prof. Dr. med. Erwin Gohrbandt.
Charged with securing the sanitary supplying and the watching over the hygienic situation by the Magistrat of Berlin.
On 12.10.1945 the allied command (the US-military administration) in Berlin dismissed the world-famous surgeon out of his office as “Stadtrat für das Gesundheitswesen” because of “his political activity in the Naziregime”, Sauerbruch was deeply grieved.
But the Soviets reinstated him as surgical superintendent of the Charité-hospital and professor in ordinary of the now so called Wilhelm-von-Humboldt-Universität in Berlin- East
Since 1948 a severe sclerosis of his brain darkened his actions operating now as before.
In 1948 when the "Chirurgische Gesellschaft in Berlin" was founded again he was elected as the chairmain.

A “Entnazifizierungs-Spruchkammer“ (court) hatte ihn als Mitläufer (declared as “hanger-on“) eingestuft, auch dort hatte er unflätige Tobsuchtsanfälle hingelegt (dirty frenzy attacks).
Sauerbruch was also Dr. med. honoris causa of the university of Upsala.

00.00.1939 zweite Ehe mit Margot Grossmann (* 14.02.1905 Dresden, † 03.02.1995 Berlin). Sie führte an seiner Seite ein ereignisreiches und erfülltes Leben. Nach dem Studium in Berlin arbeitete sie zunächst als Internistin am Martin- Luther- Krankenhaus. Den Geheimrat lernte sie an der Charité kennen. Sie assistierte dem berühmten Mediziner bei Operationen. Die promovierte Ärztin war bis zuletzt als Gesellschafterin und Beraterin einer Firma tätig, die pharmazeutische und kosmetische Präparate auf Plazenta-Basis her-stellte. Mit ihrem Mann teilte sie die Liebe zu Katzen und Pferden.
Im Umfeld von Sauerbruch tummelten sich Spione. Der bekannteste war Fritz Kolbe; der Beamte im Auswärtigen Dienst spitzelte für die USA. Seine Verlobte und Helferin Maria Fritsch war Sauerbruchs Sekretärin. Als Patient und Freund Fritschs kam Kolbe oft in die Charité und arbeitete ab 1943 mit Adolphe Jung zusammen, Sauerbruchs Oberarzt, der für die französische Résistance tätig war. Auch der spätere KGB-Spion Wolfgang Wohlgemuth war ein Chirurg Sauerbruchs. Er hatte zudem den Ruf, dass NS-Funktionäre bei ihm auch harmlose Operationen nicht überlebten.
