Excellent find. Seems like almost criminal incompetence, if the writer has it right.

Not long after this article was published, there was a follow-up by Mark Horan which settled a few matters. Beyond the incompetence of the RN Admiralty, there was the dementia of the commander D'Oyley-Hughes (hoping I spelled that right). He was a legitimate nut case.
