What is the medal in the left pocket? Is it Spanish medal?

Pretty sure it's the Spanish Order of St. Ferdinand....not sure what grade (but must be a higher degree for a General rank)....there are numerous variations of this award....according to "Vernon's Collector's Guide to Orders, Medals, & Decorations".

Gen'l Niehoff has the 1937-39 Red Cross Neck Order...

It's the Old Navarra's Shield,not the San Fernando's Cross decoration.
