At the risk of sounding argumentative; he is a Kapitänleutnant (V) - Verwaltungsoffizier (administration officer).

You are right Glenn. I was sure he was an Arzt, but I compared the sleeve insignia with Konteradmiral (V) August Böning and you are absolutely correct.

Hard to tell, but the cross looks like Mecklenburg-Strelitz. That might narrow down your search.
